The Goal of Education

This training aims to uncover the root causes of workplace accidents and near-misses and develop lasting solutions using systematic review methods. Participants will gain practical skills in effective interviewing techniques, evidence collection, documentation, and analysis. By the end of the training, they will contribute to the development of a learning safety culture throughout the organization.

Program Content

• Event types and classification approaches
• Accident investigation process and basic steps
• Evidence collection, recording and analysis methods
• Effective interview and witness interview techniques
• 5W1H, Fishbone, Why-Why Analysis, FTA applications
• Determining direct, indirect and systemic causes
• Root cause analysis (RCA) practices
• Planning corrective and preventive actions (CAPA)
• Preparation and presentation of incident reports
• Institutional learning, behavioral safety and culture development

Duration of Education

2 days (16 hours)

ISO 45001, ISO 45003, ISO 31000, BS EN 62740:2015

Education Method

Training can be delivered in both face-to-face classroom settings and online interactive sessions. The program utilizes case studies, group exercises, and scenario-based analyses. Participants will step-by-step implement evidence gathering and root cause analysis using real-world examples.

Certificate

At the end of the program, a short evaluation is made and participants who successfully complete the program are given a Certificate of Participation.

Necessary Preliminary Information

There are no prerequisites for participating in the training.

Number of Participants

Maximum of 12 people per group.

It is suitable for occupational safety experts, engineers, managers, supervisors, human resources professionals and all employees involved in incident investigation processes.

Instructor

The training will be given by internationally competent, experienced instructors.
